CareOne Senior Care is seeking a dynamic and compassionate Care Coordinator to join our growing team. We provide non-medical in-home and facility-based care services throughout all five counties of Greater Southeast Michigan. This position is based in our Farmington Hills office.

Position Overview
The Care Coordinator is responsible for overseeing all aspects of client care and case management. This role works closely with the Staffing Coordinator and Scheduler to ensure the best possible client-caregiver match, provide caregiver support and training, oversee client onboarding, and maintain high standards of care through ongoing assessments and quality assurance visits.

Key Responsibilities
Responsibilities include, but are not limited to:
Receive and manage incoming service inquiry calls; convert inquiries into in-home assessment visits
Conduct client assessments and introduce CareOne Senior Care services
Develop individualized, client-centered care plans
Establish client records and complete all onboarding documentation
Introduce caregivers to clients and families
Provide caregiver support, training, and supervision
Conduct client reassessments and quality assurance visits
Collaborate closely with scheduling and staffing teams to ensure continuity of care
Cross-train in basic office functions, including scheduling support
Participate in on-call rotation: minimum of one weekday evening per week and one weekend per month
Schedule Flexibility
This position requires the ability and willingness to work outside of standard 8:00 a.m. – 5:00 p.m. office hours on occasion, based on client and operational needs.
Qualifications
The ideal candidate will possess:
CNA certification with a minimum of two (2) years of hands-on experience; LPN licensure preferred
Minimum one (1) year of Care Coordinator or related experience
High School Diploma required; Associate’s degree or higher preferred
Strong interpersonal skills with a compassionate, positive demeanor
Excellent written and verbal communication skills, with the ability to communicate clearly and professionally with clients, families, caregivers, referral partners, and internal team members
Demonstrated knowledge of the continuum of care for seniors, including geriatric care needs across home care, assisted living, skilled nursing, hospice, and related support services
Ability to multitask, stay calm under pressure, and work efficiently in a fast-paced environment
Exceptional organization, attention to detail, and ability to prioritize competing demands
Strong problem-solving, critical-thinking, and decision-making abilities
Computer literacy and comfort with scheduling, documentation, and electronic systems
Commitment to a team-oriented culture and alignment with company mission and values
