Home Care Community Liaison / Client Acquisitions Coordinator

CareOne Senior Care, a leading provider of non-medical home care services, is seeking a passionate and results-driven Community Liaison / Client Acquisitions Coordinator to join our team. This is a field-based position focused on building referral relationships and onboarding new clients into care.

Position Overview:

As a Community Liaison / Client Acquisitions Coordinator, you will be responsible for driving business growth and client acquisition through strong community partnerships and exceptional onboarding practices.

Referral & Community Engagement

• Build and maintain relationships with key referral sources (e.g., hospitals, rehabs, physician offices, assisted living communities)

• Represent CareOne Senior Care with professionalism and compassion in the community

• Collaborate with the Agency Administrator to attend networking events, community senior events, health fairs, and professional outreach opportunities

• Support the referral process by educating potential clients and referral partners on CareOne’s services

• Identify new business development opportunities

Client Onboarding & Coordination

• Conduct in-home or facility-based client assessments

• Coordinate and complete the onboarding process for new clients

• Develop individualized care plans based on client needs

• Ensure documentation is completed thoroughly and accurately

• Conduct follow-up assessments as needed to support service continuity

Requirements:

• Minimum 1 year of experience in Home Health, Home Care, or a related healthcare field

• Sales, outreach, or community relations experience preferred — or a willingness to learn

• Strong interpersonal and communication skills

• Exceptional attention to detail and organizational abilities

• Compassionate about serving seniors and helping them age in place safely

• Self-motivated with the ability to work independently in the field

• Valid Driver’s License, reliable transportation, and ability to travel daily

• Must pass a criminal background check

• Knowledge of local healthcare providers, senior services, and care networks

• Professionalism in all written and verbal communication
